Clinical Tip No. 9: Don’t Forget to take Blood Cultures

    🛑 THE PROBLEM
    If you start broad-spectrum antibiotics, you may plan to de-escalate once you isolate the pathogen.
    But if you fail to take the necessary samples in time, you’ll have no choice but to continue with overly broad antibiotics for the entire course of treatment.

    📚 THE BACKGROUND
    In many emergency departments, due to logistical issues or simple lack of awareness, patients receiving broad-spectrum empiric therapy (such as meropenem plus vancomycin) often do not have blood cultures drawn beforehand.

    As a result, it becomes impossible to de-escalate or adjust therapy, because the causative pathogen is never identified.
    The patient ends up receiving very broad-spectrum antibiotics for too long, increasing the risk of:

    • Adverse events

    • Bacterial resistance

    • Unnecessary cost

    ✅ WHAT TO DO
    Always order two sets of blood cultures before starting broad empiric therapy.

    If the patient is sick enough to warrant meropenem plus vancomycin, they absolutely need blood cultures drawn first.
    No exceptions.
