Clinical Tip No. 7: No Antibiotics For Strep Throat???

    🛑 THE PROBLEM
    What happens if you don’t prescribe antibiotics for strep throat?
    Will your patient get rheumatic fever or another terrible complication?

    📚 THE BACKGROUND
    We all learned in medical school that untreated strep throat can lead to rheumatic fever.
    While this remains a real concern in developing countries, rheumatic fever has become exceedingly rare in highly industrialized nations, perhaps with the exception of certain indigenous populations in North America and Australia.

    Here in Croatia, in 15 years of practice, I haven’t seen a single case!
    Here is a comforting fact: We undoubtedly miss many cases of strep throat. Patients with mild symptoms often don’t even seek medical care. And yet, we still don’t see rheumatic fever!

    ✅ WHAT TO DO
    Don’t prescribe antibiotics for every case of pharyngitis “just to be safe.”
    Only treat if there is a confirmed or highly probable Group A Streptococcus (GAS) infection.

    Even when it is present, strep throat is almost always self-limiting!
